Dear Principal:

Entries are now being accepted for the Chicago Tribune's 27th annual Illinois High School All-State Academic Team scholarship, and we invite you to nominate a student from your senior class.

(Click here to view a 2012 nomination form)

The All-State Academic Team recognizes the achievements of outstanding high school seniors in Illinois. From the entries, our judges will select 70 finalists--10 from each of seven regions in the state. From those finalists, the judges will select 10 winners of the one-time $2,000 college scholarship. The Tribune will publish news articles about each member of the 2012 team as well as the names and schools of the other 60 finalists on Sunday, May 6, 2012.

Nomination forms will not be mailed this year. Instead, the school principal or his or her designee should print out this nomination form to be filled out then returned to us by mail. Entries must be postmarked no later than Saturday, March 24, 2012. You may nominate only one student; employees of Tribune Co. or its subsidiaries and their immediate family members are not eligible.

The principal or designee should complete and sign the first page of the nomination form. The nominee should complete the remainder of the form and write the essay as described. Attach a copy of the student's grade transcript for seven high school semesters (you may obscure the Social Security number) and mail the entire package to:
